In conjunction with Health Services, coordinate mental health related activities including but not limited to acquiring mental health history documentation, facilitating evaluation/treatment referral, following up with treatment provider about progress, etc. Provide therapy: individual, group, and family counseling, using agreed-upon models including TF-CBT. Work a schedule to be agreed with supervisor and that contains at least one late day a week. Be available via agency phone at weekends for crisis intervention as needed. Function as member of multidisciplinary treatment team. Participate in Team Meetings, Treatment Team Meetings, Service Plan Reviews, Family Team Conferences and other meetings.
Act as a liaison with Division of Youth and Family Justice (DYFJ) of ACS, family court and appropriate community agencies and other stakeholders. Facilitate community re-entry and in collaboration with other team members arrange for wrap-around services in the community as needed, collaborating with all aftercare services.
