Serves as a primary point of contact, assisting clients in coordinating necessary services and resources to address the client's complex needs. Case management relies on the clients to play an active role in their development. Responsibilities Attend relevant seminars/conferences. Conduct comprehensive, individualized, strengths-based, and culturally responsive backssments of each client's needs and available resources.
Empower the clients to access the resources available to them effectively. Engage each client to participate cooperatively in the development, implementation, and ongoing review of an individualized action plan so as not to foster dependence.
Monitor and document clients' progress towards their goals utilizing standardized forms and enter the relevant information into the client registry (Service Point) in a timely manner. Oversee the Comprehensive Emergency Assistant Program (CEAP). Meet with clients to perform an initial intake, including an application detailing client needs. Compile a monthly budget for each client seeking assistance. Conduct financial literacy courses. Maintain all
client files with pertinent information. Perform intake interviews and backssments via phone and in person, linking families to resources for urgent needs.
Perform outreach to identify vulnerable persons in need of services and referrals. Prepare monthly statistical reports as requested and attend monthly coordination meetings. Provide continuity of client services through case transfer or case closure. Provide, refer, or otherwise arrange for families to receive needed services and resources identified in their action plan. Verify unmet needs by obtaining records and contacting vendors. Network with other organizations to guide clients through the delivery sequence without duplication of benefits or services.
Advocate with and for clients by providing support and advocacy with governmental and non-governmental agencies and organizations when necessary. Screen applicants promptly and responsively to identify the urgency of need and direct them to appropriate services, providing accurate and timely information and referral. Work with a designated Pastoral Care Representative to schedule spiritual guidance sessions at the client's request. Qualifications Bachelor's degree in related social services field preferred; three years of case management required.
Ability to communicate effectively, both verbally and in writing. Ability to maintain ethical conduct in accordance with organizational policy and carry out business in a manner consistent with the organization's mission, values, and policies. Ability to work without close supervision on assigned duties and willingness to seek and accept supervision as needed. Bilingual in English and Spanish is a plus. Computer proficiency required, particularly in Microsoft Office. Demonstrated sensitivity to issues related to poverty and the barriers faced by low-income people. Excellent interpersonal skills, including empathetic listening.
Must complete Keep SAfe and other required training upon hire. Must have a valid driver's license and clean driving record. Prior experience with motivational interviewing and the strengths-based perspective is a plus. Unquestioned confidentiality with sensitive financial and personal information.
