Provides patient-centered care, using critical thinking skills to ensure positive patient outcomes. The RN Case Manager is responsible for care coordination, appropriate visit pattern utilization, outcome monitoring/management, resource management and episode
management. Qualifications REQUIRED QUALIFICATIONS: Licenses/Certifications: Current licensure by the State Board of Nursing in appropriate state (Illinois or Michigan) Current American Heart Association Health Care Provider BLS is required prior to start date of employment.
