About The Job Provides skilled nursing services to patients and families at end of life including backssment, planning, intervention and evaluation in their home setting. This position works during the day and assists with call schedule for nights, weekends, and holidays. The Hospice Home Care Nurse is part of the interdisciplinary team that helps patients and their families remain in their home setting during their journey.

What We Are Looking For Education [formal]: Current RN license is required BSN preferred Hospice and Palliative Care Certification preferred Experience: Must have demonstrated nursing competence in a health care setting for two years Caring for terminally ill patients/families, Hospice or home care nursing experience is preferred Special Skills and Competencies: Critical thinking skills, communication skills, ability to problem-solve and work within a team environment.

Must demonstrate the ability to multitask in a fast paced environment with flexibility and strong organizational skills Computer skills/typing skills, and the ability to use electronic methods of communication Obtain Port

Access Certification within one year of hire date Must be able to read, write and/or speak English.

Must provide proof of Basic Life Support (BLS Provider-American Heart Association or Professional Rescuer-American Red Cross) prior to employment or within 30 days of hire and maintains current status. Must provide proof of Mandatory Reporting for Child and Dependent Adult Abuse training prior to employment or within 6 months of hire and maintain current status per State of Iowa guidelines Motor Vehicle: __X__ Personal Vehicle [ select this for positions where colleagues are required to drive on a daily or weekly basis, other than to/from work] If required to operate personal vehicle, will need to have proof of and maintain valid driver's license and adequate insurance coverage on file with Human Resources All offers are contingent upon the successful completion of references, background checks, pre-employment physical, drug screen and post offer exam.
