Serves patients, families and significant others whose ages range from young children to adults What You Will Do Identifies patient's needs, determines patient's eligibility for level of care and refers appropriate services.
Evaluates SI (Severity of Illness) and IS (Intensity of Services) for appropriate level of care. Evaluates potential admissions/transfers to Mercy One Medical Center Clinton. Documents patient progress toward goal achievement. Analyzes data to formulate appropriate intervention collaborate with physicians on non-payment
denials. Participates in discharge planning, patient care conference and facilitates communication among caregivers. Demonstrates working knowledge of available community resources.
Monitors, evaluates, coordinates, supports, and reports relevant information/data related to quality-of-care issues. Understands payor systems and impacts. Minimum Qualifications Active registration for Registered Nurse - Board of Nursing, State of Iowa. Three years' experience in health care delivery. Must be BLS certificate within 3 months of hire. Baccalaureate Degree in health-related field required or completion within six years of hire. Must work well with clients and families with diverse opinions,
religious and cultural ideals and values. Must work autonomously with little direction.
Demonstrates ability to determine if client is in homeostasis regardless of living conditions or disease processes. Must possess leadership qualities including: time management, verbal and written communication skills, listening skills, problem solving, decision making, priority setting, work delegation, work organization, and customer service focus. Must possess a personal presence that is characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission and vision, goals, and values of Trinity Health.
