Makes the initial evaluation and re-evaluates the patient's nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. 1+ year experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws. 90 day commitment interval with a weekly commitment of 24 - 36 hours (combination of 8 or all 12 hours) depended on unit needs and Nursing Dir approval. Reporting to the unit's Nurse Manager, the Registered Nurse works collaboratively with members of the Care Team to ensure that patients receive exceptional, high quality, patient-centered nursing care that meets their individual needs.
Job Description: Essential Duties & Responsibilities (including but not limited to): Provides direct care to patients and makes necessary nursing judgments. Education is planned and provided according to care plan and predicted Length of Stay for every patient who is alert and able to comprehend. Family members and care givers are included in education as appropriate. a. Participates in the development of educational materials/presentations for patients and/or significant others and documents outcome in the medical record. b. Ensures that patients and significant others receive education regarding discharge and the community services available. Maintains annual mandatory education requirements, which include emergency skills and unit based competencies.
