Submits clear and appropriate documentation in a timely manner and documents patient education. Collaborating with doctors and other healthcare professionals to develop improved diets and healthcare plans for patients. Participates in case conferences to promote coordination of care. Promotes continuity of care with appropriate admissions, transfers and discharges. Keeping abreast of developments in healthcare, attend in-service training in addition attend agency meetings as required.
