implementation, and evaluation to ensure quality nursing care and to maximize the quality of life for the patients.

The RN/LPN is an integral member of the clinical team, and collaborates with other team members to meet the clinical needs of the patients.

The RN/LPN serves as a role model and mentor to others. This is a Monday-Friday or Monday-Thursday schedule, no weekends, evenings or on-call hours! of accountability and personal ownership related to teamwork and patient care Demonstrates strong critical thinking skills Demonstrates ability to coach and mentor others to achieve high quality care Position Responsibilities Ensures all care delivered to patients in the outpatient setting is within the scope and established standards of safe professional nursing practice and core care competencies.

The Outpatient Staff Nurse should be able to: Triage patients in the practice or home settings; ensuring that patients with urgent clinical issues are managed in a timely manner Track the illness status of patients who are being monitored for infectious disease and other acute conditions Perform wound care treatments Perform point of care testing within the scope of the practice's CLIA licensure In collaboration with the licensed practitioner/medical provider, provide education and care coordination e.

g. for diabetes management, respiratory disease management and other chronic diseases In collaboration with the licensed practitioner/medical provider, provide education to patient care assistants and medical assistants Provide medication teaching and assist with medication management, including but not exclusive to: Helping patients improver their ability to self-manage medication administration Serving as a liaison to pharmacies re: unit dose packaging/medication box management Obtaining appropriate refill authorizations or prior authorizations when needed.

Ensuring appropriate medication associated testing is up to date Serve as a liaison to other services such as the Enhanced living unit, SNF, HSL home care/VNA, rehabilitation services, case managers, psychiatry, to ensure that effective bidirectional communication occurs. Understand use of quality measures in primary geriatric care and participate in quality improvement initiatives Coordinate care between the New Bridge on the Charles Medical Practice and specialty services at other locations, via tracking of clinical information and facilitating communication between staff members as needed Collaborates with other members of the team (including but not exclusive to the MD, NP, SW, clinic coordinator) to meet the clinical needs of the patient and monitor outcomes of care Provides administrative coverage when needed (e.

g. when the clinic coordinator is not available). Adheres to best practice recommendations and implements evidence-based interventions to ensure optimum outcomes for patients Documents pertinent information regarding care delivered in designated areas of the electronic health records Identifies and seeks out necessary resources as needed to ensure the provision of safe and effective care Assumes accountability for his/her own learning needs and professional development Demonstrates critical thinking abilities in problem resolution Role models professional behavior Actively participates in shared governance, committees, etc.

Collaborates in the development of new work processes and systems Maintains compliance with policies, procedures, practices, regulatory matters and HIPPA compliance Participates in the collection and monitoring of quality data to ensure high standards of care: Initiates necessary reports including DPH, incident reports, concern resolutions Maintains infection control reports/line lists as needed Attends clinical meetings regarding patient care as requested Completes special projects as assigned Other duties as assigned Required Qualifications Graduate of approved school of nursing required Current Massachusetts license as an RN or LPN in good standing Previous outpatient clinic practice experience preferred Must be professional, proactive, collaborative, conscientious and results-oriented individual.

Must have an optimistic and positive demeanor, excellent oral and written communication skills, good intuition and able to adapt to changing priorities and display good, sound judgment with a sense of humor Superb organizational skills. Must have solid analytical skills. Must be creative and proactive yet disciplined, discriminating and able to streamline work volume in order to maintain bottom line efforts in midst of multi-tasking and daily re-prioritizing.
