The RN is accountable for the client backssment, care planning, therapeutic intervention, and achievement of client outcomes. Provides functional support to Home Healthcare Aides and LPNs as needed. What You Will Do Effectively and efficiently prioritizes visit content and plans visits based on client capabilities, condition, preferences (when possible), physician orders and available resources.
Accurately backsses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation. backsses the client 's condition and needs and determines the agency's ability to meet those needs in the home
care/hospice setting. With the physician, office care manager and client, develops appropriate backssment-based plan of treatment including: attainable episode goals and agency services needed to improve and/or stabilize client's medical condition and promote independence.
Participates in timely revision of the plan of treatment as appropriate. Makes appropriate referrals for evaluation/care to other disciplines and services, and assists in coordinating care with others to ensure effective and efficient care is provided. Utilizes interview, observation and evaluation in backssing clients and applies nursing judgment, consistent with practice standards, in formulating nursing interventions
and making recommendations to the physician and client. Reports changes in client condition as appropriate and in a timely manner, to the client's physician and/or Case Manager/designee, and obtains orders for changes in the plan of treatment to respond to the client's condition.
Initiates and assures implementation of procedures, orders for medication and treatments as prescribed by the physician in accordance with the plan of treatment. Accurately documents observations, interventions and evaluations pertaining to client care management and services in accordance with agency policy and procedure utilizing the point of care information system. This will include: backssments, visit reports, medication information, treatments, coordination of services, client progress or lack of progress toward goals, and contacts with physicians and agency and community providers.
Takes appropriate steps to assure timely submission of client data to the office for inclusion in the client record (by timely electronic transmission of data, or when appropriate by fax or mail). Minimum Qualifications Active registration for Registered Nurse - Board of Nursing, State of Iowa or Illinois An Illinois or Iowa (whichever is not currently held) Registered Nurse - Board of Nursing, license must be applied for within 30 days of hire and secured within 6 months of hire.
Associate's degree or diploma from an accredited school of nursing. Baccalaureate degree in nursing preferred; must obtain within 6 years of hire, effective 04/01/2012. Minimum of one (1) year experience as a professional acute care nurse preferred. Home healthcare experience preferred. Valid driver's license, current auto insurance and reliable transportation to and from work site are required. Must be comfortable operating in a collaborative, shared leadership environment.
