The Community Health Nurse, LPN will be responsible for the delivery of basic nursing care to patients in a home care settings, under minimal clinical supervision, as directed by a Registered Nurse, and in accordance with an established nursing plan of care.
Qualifications: Accredited School of Nursing LPN Program Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience Current LPN licensure or compact licensure recognized by the State of Vermont required Previous home care desired but not required.
