patient care team, which includes physician, advance practice providers, medical assistant, schedulers, and other clinical and management support staff. Responsibilities include direct and indirect patient care, responsibilities related to EPIC, patient triage (includes telephone triage), participation in quality improvement, and program and staff development.

This position requires independent critical thinking, very strong backssment skills, case management, transition of care, and the ability to appropriately triage patients with complex, multi-system medical conditions. The RN must be able to work as part of the care team that includes nurses, medical assistants, schedulers, and all

other disciplines as needed, and functions in a collaborative manner to ensure effective and timely communication with the patient, family, and other team members.

The IR Nurse Navigator provides compassionate, evidence-based, and efficient care to individuals, families, communities and patient populations. The RN's care delivery is consistent with the Oregon Nurse Practice Act, the ANA Scope and Standards of Practice, and the ANA Code of Ethics and meets the standards/expectations of a Professional Practice Model. In that model, the RN demonstrates the professional role obligations of scientist, leader, practitioner, and knowledge transferor. Professional accountability enriches the

RN's engagement as a leader in promoting an inter-professional culture of collaborative decision-making, innovation, life-long learning, and teamwork.

Interventional Radiology Nurse Navigator role is to act as a comprehensive patient follow-up resource and support for procedural and clinical intersections. The Nurse Navigator ensures evidenced-based and goal-directed patient education & proper follow-up planning/management for interventional therapies we provide through our department. This role also acts as a liaison between post-procedure outpatient needs & provider supported intervention, including triage of urgent or emergent post-procedure complications.

This is accomplished through the collaboration of the IR team to: Department Specific Responsibilities and Performance Standards: Independent nursing practice; all aspects performed within scope of RN licensure and role: Follow-up review of post-procedure care planning (labs, imaging, follow-up visits) to ensure recommendations are executed within follow-up timelines. Post procedural phone call check-ins to backss recovery and provide support for questions and concerns Manage post-procedural symptom calls and triage accordingly Provide clinic support for providers and support staff (external ordering, digitizing imaging, image procurement)Provide RN referral reviews for potential clinic patients Review post procedure charting and ensure follow-up plans are implemented according to the direction of the providers.

Collaborate with multi-disciplinary support staff to bridge any gaps in communication and management of mutual patients. Provide hospital-wide education to nursing staff about equipment and interventions placed or performed by IR. Perform suture removals for our central line patients and T-tack removal in clinic, available 1 hour daily. Coordination of FMLA, Pleur-x paperwork, Prescriptions, and miscellaneous items needing nurse review.

Serve as a general resource for patient care related questions from support staff Provide education to new inpatient/ outpatient Pleur-X patients Provide support for Oncology patients and interventions for both clinical and procedural areas, including performing Y90 procedures with the team when needed. Support the nursing team to develop staffing schedules that create a constant nurse presence as part of the team Cross train between specialties to provide comprehensive patient care to supplement team member vacations or illness.

Department Specific Responsibilities and Performance Standards: Current, unencumbered Oregon RN license Current BLS, required at time of hire Bachelor's degree in Nursing for external hires; BSN preferred for internal candidates The Registered Nurse Coordinator role depends heavily on the RN Role Components of Scientist, Leader, Practitioner and Knowledge Transferor as defined by the OHSU RN Role Description; however, the position requires advanced critical thinking within the Leader Component to meet the goals of the role.1-2 years nursing experience in the inpatient setting with a complex medical or surgical focus or in ambulatory or community based setting working with chronically ill or medically complex patients Department Specific Preferences (competencies, certifications, etc.
