The RN possesses the specialized knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness. Essential Job Functions Collaborates with members of interdisciplinary teams
to establish goals and strategies for meeting the discharge or continuing care needs with the patient. Performs and documents complete patient backssments. Initiates and maintains current nursing care plans for all assigned patients.
Identifies patient learning needs to provide individualized and comprehensive teaching. Reinforces the teaching done by other members of the rehabilitation team and other healthcare disciplines and provides resource materials for patients and families. Fosters independence in patients and families for self-care skills, self-management, decision making, and goal development Administers medications in a safe manner consistent with the State of Practice and

Acts as a patient safety advocate by participating in ongoing quality improvement in the department. Serves as a patient advocate in collaboration with spiritual care, palliative care, and ethics.
