Makes the initial visit to backss the patient's physical and emotional status, needs, and resources through observation, interview and analysis of records.
Prepares the nursing plan of care, in accordance with the physician's plan of treatment, based on problems identified in the nursing backssment and provides patient teaching using the Care Plan/Standard of Care. Interprets to the patient and family the implications of the diagnosis and nature of the treatment, consistent with the Physician's instruction. Provides emotional
support and encouragement to help the patient and family accept and adjust to the effects of disease, disability, or dying. Supervises and evaluates patient self care or care given by others.
Notifies physician and Clinical Services Supervisor of changes in patient condition. Required: Graduate of an accredited school of nursing. Preference will be given to applicants with educational preparation appropriate to the immediate or long-range needs of the agency. Current license to practice as a Registered Nurse in the State of Michigan. Minimum of one to two years of med/surg, critical care or demonstrated experience in a specialty area. Current Michigan driver's license and proof of valid
automobile insurance. Preferred: Two years hospital experience. Experience in a certified home health agency.
