  • Location: Plymouth, MA

provides coordination of palliative medical services for children and families enrolled in the Fragile Footprints Pediatric Palliative Care Program. Provides clinical backssment of children diagnosed with life-limiting illness. Collaborates with other team members and other health care providers to provide a coordinated plan of care for the child which extends across all care settings and must withstand complex, competing and shifting goals of care.

Works in collaboration with other health care providers to provide support, psychosocial education and counseling to the child and family about the child’s life-limiting diagnosis, prognosis, psychological stress, fears and anxieties, advance

care planning, advance directives, withholding or withdrawing life-sustaining therapies and options for the plans of care including end of life decisions. Provides and coordinates sibling support.

Consults with primary care and other providers to provide information about the full range of clinical resources available to the child and makes referrals to other community resources and services to support the plan of care and needs of the child and family. Collaborates with the primary care and other providers to support and promote the normal growth and development of the child and his or her siblings. Initiates and/or responds to questions regarding end-of-life care in collaboration with

other TEAM members and primary care providers. Participates as an active member of the Interdisciplinary Pediatric Palliative Team to ensure compliance to the pediatric palliative care standards and to ensure the needs of the children and families enrolled in the program are fully met.

Education/Experience Required: Recent experience with medically fragile children and families Knowledge of age-appropriate responses to illness and stress Hospice and/or palliative care experience preferred Comprehensive knowledge of the palliative model of care delivery Excellent written, oral and presentation skills Demonstrated ability at self-organization and effective documentation Demonstrated clinical and customer service approach in hospice, palliative care or related field Registration/Certification: Current Massachusetts LICSW licensure or eligible FLSA Status: Non-Exempt As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
