This includes intermittent skilled nursing services and directs, supervises, and instructs
hospice aide staff in the provision of personal patient care. Assume primary responsibility for the patient/family caseload that includes backssing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and/or holidays Under the physician's order, admits patients eligible for hospice services in collaboration with
the triage team Reports patient status and need for other disciplines to clinical leadership, attending physician and hospice physician Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and/or holiday(s) Current driver's license and automobile in good working condition with proof of auto insurance Ability to work in a field setting and exhibited ability to make sound nursing judgments; Ability to backss patient needs and formulate individualized patient care plans to meet those needs Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers Two years' experience within hospice Certified Hospice and Palliative Nursing Electronic Medical Records Experience For more details: jobs-search.

Acts as the point of contact for the insured's, vendors and public adjustor when onsite. Provides support to the outside contents evaluator assistants. Prepares field reports of the inventory capture. Uploads all digital media and completes quality check for daily field activity to maintain accuracy. Oversees all necessary maintenance of crew vehicles, including: gas, oil changes, tire rotations, and any other necessary repairs.
