In collaboration with other unit-based leadership, coordinates team huddles during the day and in the evening to review new admissions, patient progression towards goals and discharge plan, to improve access and facilitate patient transitions. Responsible for communicating, educating and monitoring changes to clinical practice and expectations including reviewing documentation, providing additional support to staff and reporting ongoing non-compliance to management. Collaborates with the leadership team in the strategic planning, implementation, evaluation, and revision of systems to support and enhance the competence, autonomy, and accountability of the nursing staff including defining nursing standards, practices, and educational needs of patient and nursing staff.
Leadership/Management, Human Resources Acts as a member of the nursing leadership team by participating in interview panels, orientation, annual regulatory and clinical competency requirements. Reports unprofessional or unsafe practice to the Nurse Manager. Assists Manager and/or designee to ensure the department complies with hospital established policies, regulatory, quality assurance programs, safety, and infection control policies and procedures.
Quality, Safety and Experience Improvements Promotes a culture of safety within the organization. Reports out on all entered Safety Intelligence Reports; Responsible for initial review and investigation and escalate serious reportable events (SRE) immediately to the Nurse Manager Standard Staffing Level Responsibilities: Coordinates the assignment of tasks and helps resolve technical and operational problems. Evaluates the impact of solutions to ensure goals are achieved. Provides effective direction, guidance, and leadership over the staff for effective teamwork and motivation and fosters the effective integration of efforts with system-wide initiatives.
Position Qualifications: ADDENDUM CARE COORDINATION Difference in Qualifications Required: Three years of direct hospital care management experience within the past 8 years. Preferred: Current Case Manager Certification. ADDENDUM EMPLOYEE HEALTH SERVICES Difference in Qualifications Preferred: in Occupational Health. License/Certification/Education: Required: Bachelor's degree in Nursing, Education, or related healthcare field.
Currently holds an active RN license in the State of Massachusetts. Maintains a current BLS instructor card Preferred: Master's degree in Nursing, Education, or related healthcare field. Experience/Skills: Required: Must have at least 3 years of clinical nursing experience with at least 1year experience in clinical preceptor and/or charge nurse role. Demonstrated leadership experience. Excellent oral and written communication skills. Knowledge of word processing, spreadsheet, and presentation computer software. Complies with requirements of accreditation and regulatory agencies by attending mandatory in-services and continuing laboratory education and participating in continuous quality improvement.
