GENERAL FUNCTIONS backsses human responses and adaptation to actual or potential health problems of the patient, family and/or significant other Institutes and initiates a plan for patient care based on the initial backssment; implements and continuously evaluates interventions; reassesses patient needs on an on-going basis.

Manages patient hand-offs at shift change and between department and facilities; utilizes documentation tools for hand-offs as required Provides every patient the defined standard of care (i. e. ; core measures, chest pain protocol) Utilizes resources efficiently and effectively Maintains safe environment Addresses patient population—specific needs (clinical) Participates in Performance Improvement Activities POSITION QUALIFICATIONS MINIMUM EDUCATION: Graduate of accredited school of nursing PREFERRED EDUCATION: Bachelor’s Degree MINIMUM EXPERIENCE: Proficient in use of computer software to include electronic patient records PREFERRED EXPERIENCE: REQUIRED CERTIFICATIONS/LICENSURE: Possession of current Nurse Permit PREFERRED CERTIFICATIONS/LICENSURE: REQUIRED CCOURSES/COMPLETIONS (e.

g.
