The position provides tele-nursing care to a diverse population of critically ill patients across the SLHS. Serves as a tele-health nurse to patients discharged from SLHS facilities to evaluate, triage, and communicate with SLHS physicians, clinical staff, Care Integration and other clinical and social support as needed. Delegates task as appropriate and assumes preceptor responsibilities
