backss patients' condition during every visit and chart observations in electronic medical record Perform evaluation tasks, including vital signs, data collection with medical history intake, and medication reconciliation, reporting findings to the patient's provider Assists outreach team
engaging patients in the community Coordinate with multidisciplinary teams necessary Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan Provide palliative care as needed to keep the patient comfortable Make recommendations for devices or tools that might improve the patient's quality of life Ability to travel to multiple patients' homes per day Prepare specimens & specimens for lab analysis.
Educate patients on general health & basic self-care procedures. Provide education and instruct patient in use of home equipment Actively participates in panel management bi-weekly clinical rounds Patient care may be performed in any setting,
including, but not limited to, shelter, transitional housing, assisted care and living facility, the patient's home or mutually designated location with privacy.
The direction provided by a registered nurse, nurse practitioner, or physician to the LVN is (must/will be) available by remotely linking/connecting and at least by telephone Assist outreach as needed, locating patients, providing explanation of services, helping to enroll difficult to reach patients.
