DETAILED DUTIES AND RESPONSIBILITIES: Provide counseling services, including backssment, treatment planning, treatment plan updates, and service documentation.
Directly consults with medical staff concerning counseling needs, as needed. Process referrals from providers and/or the Referrals Department for counseling/ therapy health needs, documenting the outcome. Consult with supervisory staff concerning social service issues, programmatic concerns and policies. Deliver individual
and group psychotherapeutic services to children, adolescents and adult FHCSWFL patients, as assigned. Provider documentation and charting of all encounters. Responsible for maintaining program logs, and reports.
Deliver psycho-educational individual and /or group services as assigned by supervisor. Use EHR to provide documentation of services rendered and charting of all encounters. Attend community functions, meetings, and provide outreach services as required. Other duties, as assigned. Requirements KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of counseling services in the healthcare setting. Knowledge of healthcare delivery system. Ability to communicate in writing and exceptional
interpersonal skills. Ability to constructively engage in teamwork efforts directed at providing health care services.
Work cooperatively and enthusiastically with co-workers and respond appropriately in interpersonal situations. Knowledge of State and Federal requirements in the provision of behavioral health services to the medically underserved. Knowledge of State health care regulations, laws and policies. Knowledge of counseling services in the health care setting, including cognitive and motivational interviewing. TRAINING AND EXPERIENCE: Graduation from an accredited college or university with a Masters Degree in Social Work. Licensed by the State of Florida as a Licensed Clinical Social Worker.
SPECIAL REQUIREMENTS: Bilingual in English/Spanish preferred. Must deliver counseling services at any of FHCSWFL locations. Must be able to provide own transportation.
