The OBAT Nurse will ensure that all data is collected and entered into the appropriate systems, and that follow up with patients to coordinate appointments is completed and documented. The position works in conjunction with the Nurse Manager

and CNO. The Nurse demonstrates excellent decision-making skill, problem solving, and attention to detail. The position is responsible for highly sensitive information, which requires a clear understanding of medical and behavioral health confidentiality.

Responsibilities include: Organization & coordination of SUD schedules, distribution of patients across sites and provider time blocks. Management of provider registry for patient receiving MAT. Be the patient contact and point person for accurate information for patients and providers regarding protocols for MOUD and patient education materials. Patient contact link for patient counseling, as well as patient contact link for primary

care. Provide support during screening, intake, induction, and stabilization & maintenance phases of patient treatment.

Participate in dual diagnosis group and peer driven groups. Conduct nurse visits for initial backssment and follow up appointments. Required to attend mandatory program meetings and trainings. Complete case management initial backssment to determine case management needs. Develop individualized case management plan in coordination with the member and provider to reach timely and appropriate diagnosis. Tracking, follow-up and reporting of all diagnostic services, diagnostic test results, referrals for diagnostic tests, tracking of results, and closing the loop for all test results.

Communication with providers, patients and the patient's families when appropriate Plan, coordinate, monitor, evaluate and document progress toward meeting goals and objectives on the case management plan. Attend any mandated continuing education/training. Maintains the treatment database with health information for those patients enrolled in the program and pertinent to any associated grants. Minimum Qualifications: Valid Massachusetts nursing license (RN or LPN)1-2 years' experience as a RN/LPNstrong understanding of substance use disorders.
