Essential Duties and Responsibilities: Provide primary care services. Work within all state and federal laws and regulations and the agreement with the collaborative physician. Advance the role of advanced practice nurses in underserved
areas through advocacy and other professional activities. Notify immediate supervisor of significant problems. Provide age-appropriate backssment and interventions in order to meet the needs of patients and/or families served Provides patient/family education, health counseling, and health advocacy services to patients and/or their families.
Be willing to work occasional weekends. Three (3) years of nursing experience, at least two (2) of which shall have been as an Advanced Practice Nurse in a related specialty, required. RESPONSIBILITIES: Provide bio-psychosocial evaluations for adults at health centers and outreach sites. Provide medication management using specified guidelines at health centers and outreach sites.
Organize group education programs, as appropriate, for homeless people. Refer clients to psychiatric and other emergency departments, substance abuse and mental health care providers, and other providers as appropriate. Counsel consumers with mental illness and substance abuse disorders on the management of their illnesses. Incorporate evidence-based practices including cognitive behavioral
therapy and self-management. Train shelter staff, as needed, on the management of behavioral health problems and other health issues. Participate in advocacy and coordination committees addressing issues related to homelessness.
Provide consultation on behavioral health to Health Care for the Homeless management and clinical staff. Train community and academic health care professionals to provide comprehensive, culturally competent behavioral health care to homeless people. Notify the Infection Control Coordinator immediately of suspected or confirmed communicable diseases that can be transmitted through routine shelter contact. Provide psychiatric treatment or consultation services to
assigned clients and families as indicated and clinically appropriate, including managing an assigned caseload and completing documentation in a timely manner.
Ensure that psychiatric evaluations and medication management documentation meet all stakeholder, payor, and regulatory expectations and include clinically appropriate diagnoses and recommendations. Utilize evidence-based practices as appropriate for the individual client and program philosophy. Ensure clients are provided with psychiatric services based on need and contractual agreements. Ensure that these services are fully integrated into the overall treatment plan for each individual client.
Prescribe buprenorphine-naloxone/buprenorphine to patients who are enrolled in the MAT program and who are in compliance with the treatment contract. Perform other duties as assigned.
