Successful candidate will
conduct initial evaluation and subsequent regular follow-up backssments of transplant recipients functioning, compliance and psychosocial needs. Provide interventions as identified in backssment to address any psychosocial problems/barriers that interfere with post-transplantation plan of care.
Provide counseling, psycho-education, groups, support, crisis intervention and other psychosocial services to patients and their families in order to help them cope with psychosocial stresses related to end stage Liver disease and adjustment to Transplantation. Assist patients through advocacy procedures and referrals to community agencies as indicated. Experience working with end stage liver or
