to meet patient and family needs. The hospice aide provides personal care and light housekeeping duties in accordance with the established plan of care. This is a full-time, 40 hours per week position, with 8:00-4:30pm hours. There is a rotating weekend commitment every 3rd-4th weekend.

It is eligible for full benefits. This position is eligible for a bonus of up to $2,500. Duties / Responsibilities: Provides personal care Observes, reports, and documents patient status and the care provided Provides support to patient, family and caregivers through on-going communication Follows basic infection control procedures Recognizes and reports changes in patient's status to RN Case Manager Recognizes

emergencies and has the knowledge of emergency procedures and their application Maintains patient and family confidentiality Education / Experience: Certified Nurse Aide certification High School Diploma or GED1-3 years of experience required.
