Participates in triage phone calls from patients, hospital staff, and physician's offices, referring physician home health agencies, referring institutions, rehabilitation facilities, clinics staff, lawyers and insurance companies.
Serves as a link between acute care and outpatient to ensure continuity of care. Facilitates completion of forms required by patients, home health agencies and DME companies. Assists in development and maintenance of schedule to meet staffing levels appropriate for safe patient care. Communicates with primary care physician regarding
patient admission and discharge. Communicates discharge needs by phone or fax to PCP office on day of discharge or ASAP after discharge. Effective patient backssment skills through the ability to prioritize and utilize appropriate resources.

Education, Experience, Qualifications: Associate Degree in Nursing required. One or more years' experience as an office nurse in a family or internal medicine practice required.
