  • Location: Naples, FL

for the purpose of identifying patients/families appropriate for extended needed backssment and implementation at admission. Participates in triage phone calls from patients, hospital staff, and physician's offices, referring physician home health agencies, referring institutions, rehabilitation facilities, clinics staff, lawyers and insurance companies.

Serves as a link between acute care and outpatient to ensure continuity of care. Facilitates completion of forms required by patients, home health agencies and DME companies. Assists in development and maintenance of schedule to meet staffing levels appropriate for safe patient care. Communicates with primary care physician regarding

patient admission and discharge. Communicates discharge needs by phone or fax to PCP office on day of discharge or ASAP after discharge. Effective patient backssment skills through the ability to prioritize and utilize appropriate resources.

Must triage call from patients, family members and physicians. Works with Hospitalist team to procure OP records as needed on admission. Revises, maintains and distributes pocket reference materials. Participates in educational development and Quality projects. Assists in data tracking and trending. Participates in Hospitalist staff developments. Assists in coordination of new services. Participates in Hospitalist Quality projects. Assists

staff in identifying variances and solutions. Assists Manager in monitoring E&M data.

Education, Experience, Qualifications: Associate Degree in Nursing required. One or more years' experience as an office nurse in a family or internal medicine practice required. Current RN licensure in the state of Florida required. ACLS certification preferred.
