The nurse navigator will collaborate with the clinical care team to make sure that care is safe, efficient, effective, timely and meets the cultural, spiritual, and ethnic needs of each patient and family.

PRINCIPAL DUTIES AND RESPONSIBILITIES: The Nurse Navigator will: 1. Be the first point of contact for newly referred patients and will make sure the patient's chart has all the necessary medical information. a) Track down records from outside hospitals/doctors (including imaging docs) b) Perform chart review/data extraction

c) Draft accurate, integrated timeline of patient's IBD history to date and fill out new patient history template in EPIC a. Initial work-up initial therapy and evolution of therapy b.

Dates and results of imaging c. Dates and results of endoscopy d. Dates and results of pathology e. Dates and results of surgery f. Dates and results of pertinent labs d) Welcome each new patient to our Center by explaining the services we offer and presenting a welcome packet to each new patient e) Give out and explains the purpose of the PROMS survey. He/she will answer any questions that the patient has. 2. Coordinate referrals and decide which services will be appropriate for each patient. a) Go through

referrals and determine which ones are appropriate for the Crohn's and Colitis Center b) Determine which patients should participate in multidisciplinary clinic vs regular clinic after thoroughly looking thoroughly looking through medical history c) Answer dedicated phone line for patients and providers who want to refer patients to the Center d) Will be physically present during multidisciplinary clinic and will help patient with coordination of services e) May be asked to help with patient paperwork/insurance related work 3.

Provide competent, compassionate patient centered nursing care based on scientific principles and the use of the nursing process and interdisciplinary teamwork.

a. Provides and maintains a therapeutic environment for patients and families. b. Articulates the professional nursing role to the patient, family and health team members. c. Identifies, facilitates, and evaluates outcomes of nursing care for an individual patient or group of patients. d. Coordinates involvement of the patient, family and health team members in patient care, including patient/family teaching and discharge planning. e. Documents relevant patient information. f. Communicates the plan of care and other pertinent information to/from other health care team members.

Promotes continuity of care through effective hand-offs. g. Helps define standards of excellence for patient care; participates in improvement of patient care services in a cost-effective way. h. Performs specific functions as described in the unit Scope of Service. i. Provides care appropriate to age group of patients on assigned unit(s) as described in relevant unit Scope of Service. j. Participates in, and contributes to, performance improvement activities. k. Integrates the six aims of performance improvement into care delivery: a. Safety - no needless death, injury, pain or suffering for patients or staff.

b. Effectiveness - care and service will be based on best evidence, informed by patient values and preferences. c. Patient Centeredness - all care and service will honor the individual patients - their values, choices, culture, social context and specific needs. d. Timeliness - waste no one's time; no unnecessary waiting. e. Efficiency - remove all unnecessary processes or steps in a process; streamline all activities. f. Equity - all care and service will be fair and equitable - the system will treat all patients equally. l. Exhibits compliance with regulatory and quality and safety requirements.

2) Participates in daily operational activities necessary for safe patient/staff environment. a. Utilizes resources efficiently to facilitate optimal patient care. b. Provides input to the Crohn's and Colitis Center Leadership c. Professional Development: i. Maintains and updates clinical knowledge and skills based on current nursing education practices. ii. Adheres to Department requirements for required training. iii. Identifies specific learning needs and goals and develops a plan to meet them. iv. Contributes to relevant committees, professional meetings, continuing educational and/or formal academic programs.

LICENSES, CERTIFICATIONS, and/or REGISTRATIONS (if applicable): Current licensure as a Registered Nurse in the Commonwealth of Massachusetts EDUCATION: Masters degree preferred Bachelors degree in Nursing required with relevant significant experience; Graduation must be from an accredited nursing program. EXPERIENCE: Specify minimum creditable years of experience and clearly indicate if preferred or required Minimum of one full year of RN experience in GI, management experience preferred For more details: jobs-search.
