In addition, the heart failure program consistently receives the Get With The Guidelines - Heart Failure Gold Plus Quality Achievement Award from the American Heart Association.

The clinic specializes in various degrees of heart failure with a focus on improving our patients' quality of life and optimizing and improving their disease process. There is a strong focus on providing guideline-directed medical therapy (GDMT) and implementing the newest evidence-based care. Clinic leadership will work with you to ensure you are supported

during your orientation process and tailoring the process to meet individual needs. What you will do You will work closely with our advanced heart failure MD and advanced practice providers in managing patient medical issues as they arise and additionally, providing our patients with the resources necessary to obtain their critically needed medication therapy.

You may provide infusion therapy for patients in the clinic requiring diuresis. In this critical role, you will be a part of a strong interdisciplinary team that includes MDs, APPs, RNs, and MAs. Your colleagues value teamwork and prioritizing quality care to our patients. Minimum Qualifications Education: Graduate of an accredited

college or university affiliated nursing program. BSN graduates preferred.

Licensure: Current Licensure as a Registered Nurse RN in Connecticut. Certification: BLS (AHA/ARC). Preferred Skills: Education: Bachelor of Science in Nursing BSN degree preferred. Experience: Acute care, Medical-Surgical, Cardiology experience preferred. Certification: ACLS and Stroke Certification preferred.
