Foster a person-centered approach by actively engaging with individuals in creating and implementing personalized care plans that align with
their goals, preferences, and abilities. Support individuals in enhancing their social and communication skills, facilitating community integration, and fostering healthy relationships.
Administer medication and medical treatments as per prescribed guidelines, ensuring documentation and adherence to health-related protocols. Conduct regular backssments to identify individual needs, strengths, challenges, and areas for improvement, and collaborate with interdisciplinary teams to develop appropriate strategies and interventions. Document and maintain accurate records of significant events, daily activities, progress notes, and incidents, adhering to confidentiality guidelines and regulatory
requirements. Advocate for individuals' rights, dignity, and choice, while also ensuring their safety and wellbeing at all times.
Respond promptly and effectively to emergency situations, following established protocols and seeking assistance as appropriate. Promote and facilitate community participation, organizing and accompanying individuals in recreational activities, outings, and appointments. Collaborate and maintain open communication with families, guardians, and other team members to ensure continuity of care and effective service delivery. Engage in ongoing professional development, attend training sessions, seminars, and workshops to enhance knowledge and skills in the field of developmental disabilities.
Qualifications: High school diploma or equivalent (Bachelor's degree in a related field preferred). Demonstrated experience working with individuals with developmental disabilities, preferably in a direct care role. Proven ability to provide compassionate care and support while fostering independence and promoting individual choice. Excellent interpersonal skills, with the ability to effectively communicate and engage with individuals with diverse needs and backgrounds, their families, and team members. Strong organizational and time management skills, with the ability to multitask and manage competing priorities.
Knowledge of state and federal regulations pertaining to the provision of services to individuals with developmental disabilities. Basic computer skills and proficiency in electronic health record systems/documentation. Current CPR and First Aid certification (or willingness to obtain). Valid driver's license and reliable transportation to travel to various work locations and community outings. Ability to maintain confidentiality and demonstrate sensitivity to the rights and privacy of individuals with developmental disabilities.
Working Conditions: The Direct Support Professional role involves working in various settings, including residential homes, day programs, community outings, and other environments as required. Maintains a caseload sufficient in number, achieving enrollment, placement and billable unit/hours goals per
month. 8. Completes and reviews service plans, billing information and case notes in a timely, comprehensive and accurate manner.
9. Ensures candidates clock in and out to aid in approval and processing of payroll. 10. Coordinates all services necessary for the program candidate to maintain program retention and/or employment, advocating for program candidates needs/services. Supervisory Responsibilities The associate in this position does not have any supervisory responsibilities. Education / Experience / Certificates / Security Clearance High School Diploma or GED, required. Bachelor's Degree in human services, behavioral health or other related field, preferred.
Minimum of 1 year direct experience required working with persons with disabilities and/or barriers to employment to identify, obtain and/or retail employment opportunities. Membership in National Rehabilitation Association and/or Workforce Development Professional/Job Seeker Services Certification, preferred. Valid state driver's license, insurance and satisfactory driving record are required. The associate in this position will be required to successfully pass both state and federal criminal background checks. plan, organize, administer and backss the patient advocate programs under the guidance of the Director of Quality Programs (DQP). The contractor shall forward requests from patients asking for downtown care referrals to the DQP for review and action. The contractor shall forward concerns from patients regarding quality of care/patient safety issues to the DQP for review and action.
The contractor shall consult the DQP for guidance and/or assistance when a task is above the contractor. The contractor shall perform all other duties as assigned appropriate to the profession. The contractor shall maintain a daily entry of each positive and negative complaint along with a resolution into a
cumulative database using excel spreadsheets, logs, and graphs IAW Joint Commission and Health Services Inspection Standards and Criteria. The contractor shall extract Ad Hoc data on a monthly basis and provide statistics to the DQP, and in turn, presents to the executive staff via monthly briefing.
The contractor shall retrieve customer service surveys completed by patients and will develop and manage the MTF patient feedback program. The contractor shall plan, develop, administer and evaluate the overall patient relations data to include survey data from internal and external surveys to include but not limited to the Department of Defense (Do D), AF, Air Combat Command (ACC), and 633
MDG. The contractor shall review and backss weekly and monthly Do D Service Delivery backssments (SDA).
The contractor shall forwards weekly and monthly survey data to patient advocates, clinical staff, and executive staff via email. The contractor shall identify positive and negative trends using statistical graphs and other performance improvement tools. The contractor shall prepare recommended action plans for approval and shall provide information to executive staff via monthly briefing. The contractor shall enter data analysis and findings into the computerized databases called Health Service Inspection MEDFACTS and Joint Commission Accreditation Model Program (AMP).
After a review of data by the DQP, action plans shall be initiated to meet statutory compliance. The contractor shall be responsible for all actions necessary for scheduling, notification to attendees, preparation of agenda and topics, identifying presenters, and recording meeting minutes for the monthly patient advocate meetings. Shall be responsible for presenting survey data analyzed as stated above. The contractor shall prepare and submit meeting minutes to the DQP later than (NLT) five (5) duty days after the meeting was held. Upon approval, minutes shall be filed appropriately.
The contractor shall hold in-service meetings with the clinics on an as-needed basis. The contractor shall conduct initial training to all newly appointed patient advocates within seven (7) duty days of receipt of the appointment letter. The contractor shall prepare and present the annual schedule of training and activities to the DQP. The contractor shall develop and conduct quarterly group patient relations training using Power Point presentations, videos, role-playing, scenarios, and other forms of training for all patient advocates. Education, Skills & Experience: The contractor is required to have a bachelor’s degree in health care and/or business/management discipline.
A Master’s degree in business/management discipline is desirable. Contractor personnel shall have a basic knowledge of fiscal management, human resources management, and computer systems and applications. The contractor shall have knowledge and understanding of the Joint Commission on Accreditation of Healthcare Organizations standards, TRICARE Military Health System, Department of Defense directives, United States Air Force regulations, policies, and local medical treatment facility regulations, policies, procedures, programs, and precedents.
The contractor shall also have knowledge of the principles and practices related to health care delivery systems, hospital organizational structure, functions, and interrelationships. Insight is required to evaluate the impact of work center operations on principles of patient care concerns. Knowledge of the priorities, goals, and policies of a military medical center is highly desirable. The contractor shall possess knowledge of fact-finding and techniques. The contractor must have a strong knowledge of established training policies, regulations, and procedures; strong instructor skills; and good management skills.
The contractor shall possess skills in exercising initiative, judgment, problem-solving, and decision-making. The contractor shall have skills in developing and maintaining an effective work environment to achieve goals and objectives. Develop and facilitate job readiness workshops, covering subjects such as workplace etiquette, conflict resolution, and career advancement. Provide ongoing support to clients upon employment, assisting with workplace challenges, conflict resolution, and skill development. Monitor clients' progress in their new positions, ensuring job satisfaction and addressing any concerns that may arise.
4. Reporting and Documentation: Maintain accurate and up-to-date records of client interactions, services provided, and employment outcomes (placements, retention rates, etc. ). Generate reports on client progress and program effectiveness, identifying areas for improvement and recommending modifications to enhance outcomes. Qualifications: - Bachelor's degree in Human Services is preferred. Supported Employment certification a plus. - Proven experience (at least 1 years) as an Employment Specialist, Job Developer, or similar role while working with individuals with disabilities. - Knowledge of the job market and employment trends, including an understanding of various industries and their related skills requirements.
- Familiarity with community resources, vocational training programs, and social services. - Excellent communication skills, both written and verbal, with the ability to establish rapport and effectively engage with a diverse client population. - Strong organizational and time-management skills, with the ability to prioritize tasks and meet deadlines. - Proficient in using technology, including databases, career backssment tools, and Microsoft Office Suite. - Ability to work independently and collaboratively within a team, demonstrating flexibility and adaptability.
