Function: Receives, reviews and coordinates patient clinical referral activity. Completes admission backssments Serve as Charge Nurse on short term rehab unit Inputting physician orders in system Assists with arrangements and confirmation of all admissions.
Coordinates the assignment of attending physician. Conducts discharge backssments Completes discharge packets Conducts immunizations and covid testing for new residents Promotes optimal communication among program staff members, facility staff, referrals sources, physiciansand patients.
Adheres to all safety and risk management protocols and programs. Participates in direct and indirect patient care activities as warranted. Participates in quality improvement activities and QAPI process to increase efficiency through improved systems and processes Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, administrators and coworkers.
Performs other duties or functions as directed. Responsibilities: This position handles review of clinical data on referrals, backsses new admits, completes EMR data entry. Requirements: High School graduate. If RN, must have a current license to practice profession in state. Must have a reliable source
of transportation. Valid driver’s license and proof of insurance.
Ability to understand, remember and carry out oral and written instructions in English. Ability to work in Excel, Word, Power Point, PCC and Nexion dashboard or have the ability to learn. Ability to speak clearly and hear well enough to communicate with others. Ability to complete assignments while having constant interruptions. Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation. At least one year of office or LTC clerical experience. Knowledge of medical terminology preferred. Ability to type. Prior experience in admissions preferred.
