and families • Provide individual, group and family treatment to patients and families as clinically indicated • Conduct group and individual orientation sessions to new patients • Will provide clinical supervision to counseling staff • Provide social work and case management services to a caseload of patients.
Complete the psychosocial history and initial treatment plan within 30 days of patient admission • Complete mental status examinations and DSM V diagnosis • Develop individual treatment plan and treatment goals and objectives. Monitor patient progress for all patients on assigned caseload • Maintain clinical records in compliance with state and federal regulatory agency requirements
and program policies and procedures. • Document all interventions and significant contacts with patient and other agencies • Participate in interdisciplinary case conferences • Act as advocate for patients with other social service agencies and make referrals and follow up as clinically indicated • Perform other duties as assigned Qualifications: • Master’s Degree in Social Work from a school of Social Work required • Requires NYS license - LMSW (Licensed Masters Social Worker) or LCSW (Licensed Clinical Social Worker) • CASAC a plus • Knowledge of Relapse Prevention, Cognitive Behavioral Therapy, Motivational Interviewing, and other evidence-based practices for substance abuse treatment preferred
The hours of this position are 10:00am -6:00pm Monday through Thursdays & Friday 9:00am – 5:00pm.
Must be able to work other shifts as needed by clinic. This is a 100% grant funded position.
