We have a need for a Part-time Social Worker; and PRN Social Worker on the BH units at this facility. The Social Worker is responsible for providing high quality patient care, displaying good skills in backssment, psychodiagnosis, treatment planning, group treatment, individual treatment, family treatment, and discharge planning as well as function as a member of the multidisciplinary team and participate in the unit community relations program.

Identifies individual behavioral health needs, providing diagnoses of substance use and mental health disorders. backsses individual risk for suicide and other safety related concerns. Communicates accurately and clearly to the individual and his/her family members/significant others the identified treatment goals. Monitors progress through sessions, discussion
with team members, chart reviews or other methods. Accurately conveys progress information to family members/significant others as appropriate. Writes concise, accurate, clear progress notes in the chart as required by policy.
Provides referrals as clinically appropriate. Building strong therapeutic rapport with individuals and their families and natural support systems. Responsible for providing evidence based counseling/therapy to individuals. Performs short term counseling and crisis intervention as necessary. Coordinates with community partners in the provision of services. Completes required documentation in a timely manner that includes biopsychosocial backssments, treatment, safety
and discharge planning. Demonstrates commitment to the mission of the agency.
Keeping detailed and concise records of each individual during the course of their treatment utilizing an electronic health records system.
