Sustains and increases referral sources with hospitals, nursing homes and other community sources. Documents visits and phone calls with patients, families, and referral sources. Conducts discharge planning of hospital
patients that are being transferred home or admitted onto care with St. Croix Hospice. Mentors and trains new Registered Nurses. Attends Interdisciplinary Care Meetings regarding patients and families in the hospital.
Establishes and maintains relationships with hospital Case Mangers, Social Workers, and Physicians. Other duties may be assigned or required for the performance of this position. Qualifications Must have current Registered Nurse (RN) license in state(s) of practice. Graduate of an accredited school of nursing required. Associate degree required. Bachelor's degree preferred. One to two years of recent acute care experience in an institutional setting, and one to two years
of recent experience in home care preferred. Ability to pass DHS background study.
If Driving: Valid State Driver's License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required. Knowledge, Skills, and Abilities Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills required. Ability to effectively plan and organize, while managing competing priorities, required. Ability to effectively comprehend and utilize an electronic health record required. Ability to read and follow written instructions, and concisely document care given within established timeframes.
Ability to demonstrate ongoing competence in nursing skills per St. Croix Hospice competency checklist required. Self-directing with the ability to exercise independent judgment and work with little direct supervision required. Proficient in the English language. Able to read, speak, and write English in a clear and concise manner required. Understands hospice philosophy, secure with issues of death/dying. Comfortable providing specialized care to the terminally ill. Displays empathy for the needs of the ill, injured, frail and impaired.

and ensures dish washing products are maintained at par level. Remove dirty dishes from guest tables, using proper cleaning method.
Make sure all tables are set correctly according to AURO Hotels and Brand standards. Properly set up server station per meal period; Keep server station clean and clear of dishes, glassware, etc. Reports all incidents (compliments/complaints/technical issues) to Manager. Controls operation of his/her station. Insures proper execution of service between team members. Observe diners to respond to any additional requests and to determine when meal has been completed. Help in restaurant/outlet/kitchen when extra duties are assigned.
Be familiar with equipment and its location Clears and resets tables at conclusion of each course.
