POSITION SUMMARY: Carries a caseload of high risk/ high utilizer patients and interacts with the Physician, patients and family as needed. Serves as a resource for the care team through support, education, and clinical expertise. Provides a network wide focus to the coordination and management of patient care. care. EDUCATION, TRAINING, EXPERIENCE, CERTIFICATION AND LICENSURE: Candidate will be a NYS Registered Nurse. Minimum of bachelor's degree in nursing or other heath related field with one to two years progressive experience in the field, master's degree preferred.
Previous discharge planning, home care, and/or managed care experience desired. Participates in orientation, education conference, updates and maintains knowledge and skills related to specific areas of practice. SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS: Organizational, management and leadership skills. Competent in BLS/CPR rescues. Excellent oral and written communication skills. Demonstrated ability for problem solving
and conflict management. Must be able to work both independently and as part of a group.
