Position Summary: The MSW is a qualified professional person who provides medical social services to clients in the home following the physician's orders and under the supervision of the appropriate supervisor.

Qualifications: Masters prepared graduate of a school of social work accredited by the Council of Social Work Education Minimum of 1 year experience in a health care setting (hospital, clinic, rehabilitation center) where the team approach to treatment is

utilized A. C. S. W certification or be in the process of acquiring certification is preferred, but not required Licensure in good standing with the state Ability to work independently with minimal supervision Exceptional communication skills with focus on quality patient care and customer satisfaction Is flexible, cooperative, team player in fulfilling all obligations Position Responsibilities: Provides rehabilitative and supportive casework geared to restoring clients to their optimum level of social and health adjustment.

This includes assisting clients and their families to understand, accept and follow medical recommendations. Develops plan of care with RN case manager, physician,

client and family. Helps clients utilize the resources of their families and the community.

This may be accomplished by either referring the clients to resources or acting as an intermediary on behalf of the clients in their dealings with other health and welfare agencies. Assists clients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care. These range from counseling members of the client's family to assisting clients with admission to a nursing home. Consults with the physician and other members of the health team for the purpose of assisting them to understand significant social, emotional, and environmental factors related to the client's health problems.

Performs backssments and reassessments that reflect establishment of specific, measurable short and long term therapy, counseling, and community resource planning. Prepares clinical/progress notes on the day of the visit and documents in the clinical records weekly; provides summaries and re-evaluations if indicated. Attend case conferences as needed Performs supervisory activities of the Licensed Social Worker. During supervisory visits, exhibits full compliance and verbalizes knowledge of agency policies including but not limited to patient education, infection control, privacy, and age-specific behaviors for clients Participates in staff developments activities and in-services education Assist in the development and revision of the physician's plan of treatment Participates in discharge planning and in-service programs; Acts as a consultant to other agency personnel Communicates effectively with all providing care Confirms, on a weekly basis, the scheduling of visits with the Clinical Support Coordinator to coordinate necessary visits with other personnel For more details: jobs-search.
