JOB and UNIT SUMMARY: The admission nurse assumes primary responsibility for the admission of patients into the hospice program. The admission process includes the backssment planning and implementation of the plan of treatment. The admission nurse ensures admission to the hospice program and smooth transition and hand off to the IDT. Communication with the hospital physicians and other care providers is essential. Responsible

QUALIFICATIONS: Graduate from an accredited nursing program2-3 years med/surg nursing experience.
