Administer oral/IM/IV/SQ/Inhaled medications. 16. Coordinates delegates and/or provides all other patient care activities. 17. Coordinates in-patient admissions and ER referrals. 18. Acts as a patient advocate. 19. Documents according to standards of care. 20. Promotes and provides for patient safety in accordance with state and federal guidelines. 21. Participates in the departments performance improvement activities.
22. Provides presentations to care team members. 23. Ensures all staff participates as a member of the patient care team, participates in daily huddles and attend regular scheduled practice team meetings. 24. Attends in service and continuing education programs. 25. Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others. 26. Informs and educates patient/family to enable participation in their care and decision-making. 27. Evaluates the effectiveness of interventions and patient responses and outcomes to care.
