an on-going plan of care for services to Home Care Agency clients; and supervising the Home Care Agency aides in the implementation of the plan of care. Essential Duties and Responsibilities: Responsible for maintaining RN licensure. Responsible for initiation and implementation of a plan of care to Home Care Agency clients.
Also serves as a back up to Independent Living Nursing services. Responds or follows up on Emergency Calls and assists in Nursing Clinic as required. Participates in the rotation of on-call for the Home Care Agency. Insures initial backssment of client needs, care plan development, and appropriate follow-up backssments are accomplished. Must be available for consultation
as necessary. Monitors any significant changes in clients status and ensures proper documentation and communication with healthcare team. Performs ongoing review and updates of each clients care plan.
Understands nursing assistant scope of practice and communicates with Home Care Agency aides to ensure continuity and quality of care. Collaborates with the Home Care Agency Director to develop ongoing in-services and training for Home Care Agency staff. Collaborates with the Home Care Agency Director in hiring and orienting Home Care Agency nursing assistant staff. Participates in Client Care Review Committee (CCRC) to identify changes and/or concerns with residents and possible home care
needs. Collaborate with various TLC team and community professionals to determine clients safety and ability to maintain at current level of care and location vs.
need for a higher level of care following a decline in a clients condition. Conducts supervisory visits to comply with regulatory requirements and maintains the supervisory visit list. Provides counseling to clients and families as needed and refers specific problems to the appropriate TLC employees as necessary. Completes all necessary documentation on current Home Care Agency clients. Works closely with TLC Home Care Agency director and scheduler to ensure proper care is provided to all clients.
Responds and reacts in a kind, polite, and professional manner to clients, families, visitors, coworkers, and supervision. Responsible for protecting any confidential information concerning employees, clients, family, and/or the organization. Must demonstrate knowledge of and adhere to all Safety procedures and facility Fire, Disaster, and Bomb Threat procedures. Adheres to all organizational, departmental, and nursing procedures in the performance of position responsibilities. Must attend organizational or departmental meetings and/or in services as scheduled. Participates in rotating on-call responsibilities for the Home Care Agency.
May be required to perform other duties in keeping with the nature of this position which will not significantly alter the job requirements Education and/or Experience: RN with active and unrestricted NC nursing license.
