Primary Clinician for an assigned caseload of community and/or HOC inmate medically complex inpatients throughout their hospitalization. Develop and coordinate disposition plans in conjunction with community patient, their supports, and treatment team, considering developmental, racial, cultural, and ethnic, gender, interactionual

orientation and gender identity issues that impact the patient's aftercare needs. Provide time-sensitive crisis intervention, suicide screening, and psychotherapeutic services to community and inmate patient populations.

Completes timely & accurate documentation in the electronic medical record according to LSH, DOC, and TJC standards and policies. Gathers and utilizes clinical data to aid in development of the ongoing psychosocial backssment and discharge planning process. Clinical documentation includes but is not limited to initial psychosocial diagnostic evaluations, patient contacts, response to treatment, discharge planning, guardianship affidavits, crisis interventions. Serves

as liaison between LSH and other public and private agencies and collaborates with community providers in the continuum of care as per TJC standards regarding guardianships, legal status, entitlements.

Participates in Social Work Addiction Department Performance Improvement activities. Contributes to the enhancement of the therapeutic milieu by active participation on committees and activities by providing both formal and informal teaching/training programs, including but not limited to: new psychotherapy techniques, special interventions and clinical management. Awareness of addiction, chronic illnesses, disabilities, and/or end-of life care. Comprehension of community resources and how to access them for patient aftercare and discharge planning with the ability to initiate discharge planning upon admission.

Interpret and coordinate treatment plans among a variety of human service personnel and delivery systems. Utilize supervision effectively, i. e. Can teach clinical social work methods and techniques to social workers and students and to communicate the principles to other personnel. Understand, explain, and apply the provisions of the laws, rules regulations, policies, procedures, specifications, standards, and guidelines governing assigned unit activities.

Exercises discretion in handling confidential information in compliance with hospital standards and HIPPA regulations. Can communicate efficiently both verbally and in writing. Located in the heart of Boston's Jamaica Plain neighborhood, the Shattuck Hospital includes 248 inpatient beds including 95 Psychiatric beds and 29 Correctional Health beds. In addition, the Shattuck has a full outpatient clinic with many specialties that offers treatment to patients of the Department of Mental Health as well as Department of Correction and House of Correction inmates and community patients. The Shattuck hospital is on the forefront of substance use disorder treatment including a Suboxone clinic serving 200 patients.

In addition to direct medical services LSH hosts and supports an array of services located on the campus, serving patients struggling with addictions and homelessness. #REQUIRED WORK EXPERIENCE: At least two years of full-time, or equivalent part-time, professional experience as a clinical social worker after earning a Master's degree in social work.

A Doctorate in social work, psychology, sociology, counseling, counseling education, or human services may be substituted for the required experience on the basis of two years of education for one year of experience. A Master's or higher degree in social work is required. Licensure as a Licensed Certified Social Worker or Licensed Independent Clinical Social Worker by the Massachusetts Board of Registration in Social Work is required. Based on assignment, travel may be required. Those employees who elect to use a motor vehicle for travel must have a current and valid Massachusetts Motor Vehicle Operator's license or the equivalent from another state.
