cause analysis and perform clinical intervention through the development of a care management treatment plan specific to each participant with high level acuity needs Diagnose and provide individual, group, and family therapy as needed for participants Manage participants / job seekers facing the stress of the job search and going through difficult times Manage and coordinate mental health care for participants with severe mental illness who have high rates of behavioral health utilization and/or severe psychosocial vulnerability Coordinate with internal and external resources to meet identified needs of the participant's care plan and collaborate with providers Provide pre and post backssments

for screening of mental health concerns, intervention planning, and participant referrals to partner agencies Assess participants to recommend activities and behavioral routines to meet the social and medical needs of participants and their families Provide support and/or intervention and assist participants in understanding the implications and complexities of their current medical situation and/or overall personal care Ensure access to supportive services and retention services for participants Maintain and compile ongoing data in all required systems Maintain complete, accurate, and timely case notes and related records in manual and/or electronic systems Create, update, and ensure completion

of plans according to contract deadlines Provide detail and summary reports as needed Able to work independently and be a self-starter while remaining in communication with Project Director Ensure the confidentiality of participant information.

Protect and secure Company equipment and property Support Equus Workforce Solutions goals and objectives in meeting performance improvement targets for various initiatives, data analysis that supports care management, standardized plan of care expectations, and perform other duties as assigned Qualifications Must be a Licensed Clinical Social Worker or Licensed Medical Social Worker Requires a Master's Degree in Social Work, Psychology, Counseling, Rehabilitation, or other relevant field that provides a foundation to receive a license as required of the position Requires 2+ years of experience in behavioral health management and/or acute behavioral health care setting Requires excellent organizational, problem solving, critical thinking, time-management, verbal, and written communication skills Must be able to interact with individuals of all cultures and levels of authority One year minimum of supervisory experience preferred Experienced with Microsoft Office programs with exceptional computer skills Must have a current valid Driver's License and automobile insurance in good standing Moderate travel required About our Line of Business Equus Job Corps Workforce solutions has a deep-rooted history in the federally funded Job Corps program.
