This is a grant-funded position Position Overview: Under the supervision of the Refugee Program Nurse Coordinator, the Licensed Vocational Nurse (LVN) Navigator will provide safe, compassionate nursing care as a member of the profession, provider of patient-centered care, patient safety advocate and member
of the health care team. The Licensed Vocational Nurse with a focus on patient safety, is required to function within the parameters of the legal scope of practice of a LVN, and in accordance with the federal, state and local laws; rules and regulations; and policies, procedures and guidelines of HCPH.
Duties & Responsibilities: Serves as a liaison for the newly arrived refugees between HCPH Refugee Health Screening program and the Local Resettlement Agencies and/or service providers, social services, and other healthcare providers Provide and explain laboratory results to patients and their families, and supplies individualized health education and counseling in a patient-centered manner.
May be called upon to present group health education sessions when indicated.
Assist with control of patient flow through the facility during clinic visits, including discharging patients from the clinic following the medical providers' care plan and provides appropriate patient education. Monitors, tracks, schedule and follows up on abnormal test results/findings/missed appointments by telephone, postal correspondence, or home visits Ensures transportation and translation services are provided for the patients at their appointment Assists with patient screening (i. e. drawing blood, testing urine, taking/recording vital signs, heights and weights, vision and hearing backssment)Administers/dispenses prescribed medication according to shop D regulations Give age-appropriate immunizations, stay up to date with all CDC vaccine schedule changes, and assists the nursing team with providing CDC recommended vaccinations at all times and serves as a vaccination coordinator alternate for the Vaccine for Children's (VFC) Program with the Texas Department of Health Services (DSHS)Assist in performing data entry and electronic data processing by using the program EMR, Microsoft Excel, and any other related software.
