Provides routine health screenings and backssments for individuals experiencing homelessness. Assists with the planning and coordinating of services with other agencies to provide resources such as shelter, permanent housing, food, primary care doctors, and all other identified non-medical and health related services. backsses the patient's
physical needs and arranges for services to meet those needs. Works with great empathy, compassion, and work with a high regard for human dignity, be culturally competent, and be sensitive to and understanding of the specific needs of individuals experiencing homelessness. Works to improve the over all health and well-being of individuals experiencing homelessness with the goal of lifting them out of homelessness. Acts as a resource, a contact person, an advocate, a problem solver, while working in an interdisciplinary team to respond to the basic needs of individuals experiencing homelessness.
