Job Summary: Under the supervision of the Director of Community Well-Being the Community Outreach Nurse will Completes nursing backssments and provides nursing care including medical backssments, wound care, skin backssments, and vaccinations out in the community to individuals experiencing homelessness.

Provides routine health screenings and backssments for individuals experiencing homelessness. Assists with the planning and coordinating of services with other agencies to provide resources such as shelter, permanent housing, food, primary care doctors, and all other identified non-medical and health related services. backsses the patient's

physical needs and arranges for services to meet those needs. Job Requirements: Education and Work Experience: Bachelor's Degree in Nursing (BSN): Preferred Two years' clinical nursing experience: Preferred Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Required Valid Driver's License (DL) and must be at least 21 years of age or older: Required Essential Functions: Administers care where individuals experiencing homelessness frequent.

Conduct health backssments, provide brief interventions, triage patient and provide support in navigating health and social service systems.

Provides services such as referrals to other providers, community health resources, and provide health instructions. Works with great empathy, compassion, and work with a high regard for human dignity, be culturally competent, and be sensitive to and understanding of the specific needs of individuals experiencing homelessness. Works to improve the over all health and well-being of individuals experiencing homelessness with the goal of lifting them out of homelessness. Acts as a resource, a contact person, an advocate, a problem solver, while working in an interdisciplinary team to respond to the basic needs of individuals experiencing homelessness.
