well. As the Home Health Network Liaison, you will generate appropriate patient referrals across all Stonerise Home Health services from existing customers and continue to grow the number of referrals over time by establishing and maintaining professional relationships with all referral sources.
Some of your primary responsibilities include: Generating referrals for home health by building relationships with physicians, long-term care, independent and assisted living facilities, and other community resources Maintaining, developing, and supporting other Home Health services offerings, including but not limited to Home Health, Home Elder/Private Duty services, and Stonerise skilled nursing
