Identify complete, accurate, and logical OASIS data
and appropriate sequencing of diagnoses. Develop a care plan based on patient needs, physician orders, nursing backssment parameters and identified goals that are respectful of patient, family, community, and agency resources.
Implement the care plan and utilize nursing judgements to modify plan as needed. Evaluate for and promote a safe environment for the patient and complies with JCAHO National Patient Safety Goals. Meet requirements for OASIS lock dates, re-certifications, and discharge timeline. Communicate with other health care professionals both internal and external to the organization in a manner that assures care plan coordination as well as continuity of care. Qualifications
