The Nurse will also provide support functions

Provide telephone and in person triage independently within scope of practice and make appropriate clinical decisions. Provide basic health screenings, which may include wound care and other medical backssments. Understand and follow up on abnormal results of labs upon consultation with Licensed Medical Professional. Coordinate with community providers to obtain
needed medical support such as caregivers, durable equipment purchases and maintenance.
Coordinate and consult with PCP, medical specialists, hospitals, rehabilitation programs and other services on behalf of mutual patients. Provides skills training to patients to monitor and effectively manage medical conditions such as diabetes, COPD and Hepatitis. Administers medications and injections as prescribed by LMP. Screens participants for medication side effects and medical conditions. Provide individual and group skills training regarding nutrition, medications, and physical wellness as components of mental health recovery. Participate in training and meetings.
Provide education and training to team members regarding medications and potential side effects. Provide community-based outreach and services to current and potential participants. Complete required documentation within the specified timelines stated below under “Documentation Requirements. ” Education: Must meet the following qualifications: Licensed as an LPN or RN in the state of Oregon. Experience: Experience to demonstrate the competencies to identify precipitating events: 2 years’ experience in professional nursing field work. (required) Experience working with adults is (preferred) Certificates, Licenses, Registrations: Valid Oregon driver’s license, clean driving record.
