Nursing Process 1.1 The Clinical Nurse I (CN I) utilizes the nursing process of backssment, planning, intervention and evaluation while caring for our patients maintaining confidentiality and dignity. The Clinical Nurse provides safe, effective evidenced based care tailored to the individual needs of the patient and family. As a core member of the care delivery team, the Clinical Nurse partners with the team to facilitate the coordination of patient care at the unit level utilizing the principles of patient/family centered care. 1.2 The clinical nurse uses a 5 step process to deliver high quality comprehensive and individualized care incorporating: Patient backssment Problem identification Establishment of desired outcomes Implementation of interventions Evaluation of care with necessary revisions2.

Clinician Patient Relationship 1 Demonstrates a holistic and caring approach throughout the nurse patient relationship. Clinical Expertise/Problem Solving1 Collaborates with members of the multidisciplinary team to support the work of the team, unit activities, and the accomplishment of positive patient outcomes. Collaboration/Teamwork4.1 Delegates responsibilities to appropriate members of the care delivery team based upon an understanding of others' roles, scope of practice, knowledge, and capabilities to ensure optimal utilization of personnel.

4.2 Contributes to a positive work environment by being responsive to the need for change. 3 Maintains a collaborative work relationship with staff members through respect and recognition of cultural and human diversity. 2. Uses resources in a fiscally sound manner.1 The Mission Control Field RN is responsible for providing high-quality patient care to various patient populations within the framework of the YNHHS Home Hospital Model and includes the following key elements: Utilizing monitoring, backssment, planning, intervention and critical thinking skills to care for acute care patient populations in the home Navigating patients and families across the continuum of the YNHHS Home Hospital phases of care Monitoring and responding to biometric data per YNHHS Home Hospital protocols Conducting patient care visits as directed by the care team and/or protocol Ensuring all interventions in the patient's care plan are implemented efficiently with a patient centered approach Leveraging the resources and expertise of the YNHHS Home Hospital team and supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation Adhering to the clinical protocols and be willing to obtain the necessary training to provide care within the context of the providing evidence-based care Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts Must have a valid driver license.

Qualifications EDUCATION: Graduation from an accredited School of Nursing. All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire. EXPERIENCE: Recent 3-5 years of acute care nursing experience required.

Care Management experience a plus. Excellent communcations, interpersonal, coordinative, and organizational skills. LICENSE: RN License and a current registered nurse in the State of Connecticut.
