This position is limited and funded through a 24-month grant from the Department of Developmental Services and in partnership with Tri Counties Regional Center. Do: Program Coordination Oversees PUENTE team and supervises a Specialist. Maintains a relationship with the Regional Center liaison who supports referrals, resources, and training.
Oversees collection of monthly and quarterly reporting data for the grant; works with the VP of Strategic Initiatives to complete all grant documentation. Reviews referrals for eligibility; performs intake and orientation, if needed, for individuals accepted for services. Establishes and maintains files for each individual and monitors individual progress. Outreach Develops outreach plans to address the needs of the community; conducts community mapping and identifies areas to engage in outreach events; coordinates
participation in and attends community education events, fairs, and festivities to provide information on developmental disabilities, the Regional Center system, and Path Point services.
Conduct community outreach and present information to key stakeholders. Collects and reports data from each activity, following program guidelines; gathers feedback from individuals and families to inform ongoing program evaluation and identify areas for improvement. PUENTE Collaborative Oversees the PUENTE Collaborative which includes community partners, families, and individuals that share their experience about the process. Recruits members, manages the group and provides administrative support.
Plan and coordinate meetings and presentations. What We're Looking For: Minimum of one year of experience working with individuals with disabilities required Bilingual Spanish-English and bi-cultural Knowledge of multicultural community resources Strong interpersonal communication, public speaking, and presentation skills Demonstrated organizational skills and ability to multitask Ability to establish and maintain cooperative relationships with professionals and community groups Strong working knowledge of California Regional Center services preferred Familiar with Person-Centered Thinking and able to use associated tools Must have a valid California Driver's License and satisfactory DMV record Must receive a criminal record clearance through the DOJ and pass pre-employment health and TB screenings Culturally sensitive and aware of differences in employees based on cultural backgrounds; aware of one's own culture and how someone's background may lead to biases; has the skills and resources needed to work with employees from all different cultures and the ability to avoid making biased assumptions based on someone's culture.
