Coordinates admissions, evaluations, resumption of services, and discharges and the required documentation with the appropriate disciplines, under the direction of the DON/Administrator. Relays information between assigned homecare disciplines regarding change orders, frequencies, skills, disciplines, discharges, transfers and other patient-related changes.
